If so, there is a form that your primary care provider can fill in to explain why more dispensing fees are needed. I am a primary care provider and have completed this form for a client who requires medications to be blister packed and dispensed monthly. The increased number of dispensing fees was covered after I completed the form with rationale as to why it is required. "
It's a decrease in both the number of times they will dispense PLUS they decreased the cap on the amount per pres:cription. So they will cover less times, but they won't pay the full amount when we get a bigger order (we usually get 3-month supplies for the drugs we are able to, which is cheaper if you divide it over 3 months, but upfront costs more than a single month).
It's been completely asinine.
