Lean Ground Beef

I've been very annoyed with the last couple batches of "lean ground beef" that I have purchased from a chain store. It has been cooking up as if it was "regular". The amount of fat in the pan was practically boiling the meat vs browning it! Where is a good place to buy lean ground beef, at a decent price, that actually has a meat content? Also- is there any federal guidelines as to what ratio of fat to meat makes a difference between regular/lean/extra lean ground beef?
