Does this sound like your issue? There is 5 or so scenarios on the network about various fan issues. But need more information. CEL previous codes etc.

Customer Concern:
The Check Engine Light (CEL) is on with a code P1289 that resets immediately. Also the cooling fans run all the time with the engine running and the message center in the dash says the engine coolant temperature is out of range.
Average Reported Mileage: 31000
Tests/Procedures:
1. A code P1289 is indicating high resistance or open circuit in the Cylinder Head Temperature (CHT) sensor circuit.
2. The CHT sensor is mounted in the rear cylinder head towards the passenger side under the intake manifold. However, there is a 2 wire connector in the same area that can normally be accessed for testing. The two wires in the connector are Blue/Grey (BU/GY) and Green/White (GN/WH). The GN/WH wire should be a good ground. The BU/GY wire is the signal wire from the sensor and should read close to 5 V with the sensor disconnected, and read about 3 V at 70 degrees F with everything connected. Check the voltage on this wire to see if it reads over 4.5 V with everything connected. If so and the GN/WH wire is a good ground, the CHT sensor or the short wire harness to the CHT sensor is open.
