Community Health Nurse (RN/LPN)

Location: Sioux Valley Dakota Nation, MB

Position Summary:

The Community Health Nurse (RN/LPN) provides comprehensive nursing services to support the health and wellbeing of community members of Sioux Valley Dakota Nation. The position focuses on health promotion, disease prevention, community outreach, primary care support, and chronic disease management while ensuring services are delivered in a culturally safe and community-centered manner.

Working as part of the Sioux Valley Dakota Nation Health Centre team, the Community Health Nurse assists in implementing community health programs, supporting clinical services, providing health education, and delivering preventative care. The role requires collaboration with physicians, nurse practitioners, allied health professionals, Elders, leadership, and community members.

The Community Health Nurse may be responsible for delivering care in clinic settings, community settings, and through outreach services, including home visits. All duties are performed in accordance with professional nursing standards, provincial scope of practice, and Sioux Valley Dakota Nation policies.

Key Responsibilities:

  • Assist in the planning, implementation, and evaluation of community health programs aimed at improving the overall health of Sioux Valley Dakota Nation Members.
  • Deliver health promotion initiatives that address community health priorities such as: Diabetes prevention and management, cardiovascular health, mental wellness, substance use prevention, nutrition and health lifestyle education, injury prevention, men's and women's health clinics, prenatal/breastfeeding initiatives.
  • Participate in community health assessments and contribute to identifying emerging health needs.
  • Support community-based initiatives that promote wellness, resilience, and prevention.
  • Assist in organizing health fairs, awareness campaigns, screening days, and community wellness events.
  • Conduct patient assessments including vital signs, health history, and symptom evaluation.
  • Assist with clinical procedures and treatments under the direction of a physician or nurse practitioner when required.
  • Support maternal and child health programming including prenatal and postnatal care support.
  • Assist in providing well-baby and child wellness services.
  • Support community immunization programs in accordance with Manitoba and Federal public health guidelines.
  • Maintain accurate immunization records.
  • Assist in monitoring and reporting communicable diseases.
  • Provide education on infection prevention and public health practices.
  • Participate in outbreak response activities when required.
  • Assist in community-based management programs for chronic conditions such as: Diabetes, Hypertension, Obesity, Respiratory disease.
  • Provide health education and support to encourage self-management of chronic conditions.
  • Coordinate follow-up appointments and referrals as necessary
  • Conduct home visits to support individuals with health concerns or limited mobility when appropriate.
  • Provide nursing support for Elders and vulnerable community members.
  • Assist individuals and families in accessing appropriate health services.
  • Deliver outreach programs to increase access to care for community members
  • Support community workshops on health-related topics including: Nutrition and healthy eating, Physical activity, Mental wellness, Harm reduction, Chronic disease prevention.
  • Promote healthy lifestyle choices through culturally appropriate education strategies.
  • Provide services that respect Dakota culture, traditions, and values.
  • Work collaboratively with Elders and traditional knowledge keepers when appropriate.
  • Foster positive relationships with community members and leadership.
  • Support integration of traditional wellness practices alongside clinical services where appropriate.
  • Maintain accurate and confidential client records in accordance with privacy legislation and health centre policies.
  • Work closely with physicians, nurse practitioners, mental health workers, home care staff, and other healthcare providers.

Qualifications:

  • Registered Nurse (RN) with Bachelor of Science in Nursing OR
  • Licensed Practical Nurse (LPN) diploma from an accredited program.
  • Current registration and good standing with either:
  • College of Registered Nurses of Manitoba (CRNM), or
  • College of Licensed Practical Nurses of Manitoba (CLPNM).
  •  Experience in community health, primary care, or public health preferred.
  • Experience working in First Nations or Indigenous communities is considered an asset.
  • Experience in health promotion, chronic disease management, or community outreach is beneficial.
  • Strong clinical nursing assessment skills.
  • Knowledge of community health and public health principles.
  • Ability to provide culturally respectful and community-focused care.
  • Strong communication and interpersonal skills.
  • Ability to work both independently and within an interdisciplinary team.
  • Effective organizational and documentation skills.
  • Knowledge of infection prevention and control practices.
  • Ability to adapt services to meet community needs
  • Valid Manitoba driver's license.
  • Criminal record check and child abuse registry check.
  • Ability to travel within the community for outreach or home visits.
  • Willingness to participate in community events and health programming.

Working conditions:

  • Work is primarily based at the Sioux Valley Dakota Nation Health Centre.
  • Responsibilities may include outreach, home visits, and community program delivery.
  • Occasional evening or weekend activities may be required for community events.
