Unable to find information as to whether a consent form is required by a senior for the covid vaccine booster shot.
Perhaps someone who has already obtained the booster can provide this information.

A signed consent form is needed for every dose of the Covid vaccine.

Already got ours and you need a consent form. You can print it off from the site

https://www.gov.mb.ca/asset_library/en/covid/covid19_consent_form.pdf

If you are getting it the pharmacy and can’t print it off go 5 minutes early and fill it out there, you will need health card and ID . At Safeway there were a few people filling out their forms while waiting.
