Go to the Service Canada office on Richmond Ave (near Boston Pizza) and let them know you wish to sign up for the dental plan. They will get you logged into their computers on the necessary form. You complete the form, send it from there and the staff will print off a copy of the completed form for your records. Then you wait for a letter advising you that you are registered and you will receive a CDCP card which you present to your dental office.
