Problem with my dog: hELP!!!

HI we have a shitzu/cocakpoo who licks his private till it bleeds sometimes (took him to the vet, they say those dog lick alot)No medical problem, well now this is the 2nd time in 3 weeks that he's got his neutered penis stuck out so far it wont go back in. I paid $150 to fix it the last time, I can't get in myself and I can't afford that amount every time he has a behavioral problem and wants to lick/suck till it becomes medical! HELP what do I do with him???
