You can look online for free contract templates. Policies and contracts are so individual depending on the rules the home provider wants to have in place, their personality, the way they want to run their business and their view on things like routine, play styles, disciplinary actions etc but at least the templates will give you an idea of what to have in the contract.
Things that I include in mine are of course the basics like hours open, rate, holiday fee and the consequences of paying or picking up late. Other things that I include are my behaviour management policy, medication and accident policies, what I expect at drop off, pick up and nap times, my lunch and snack routine, things like that. I also have policies for toilet training and a list of possible reasons for termination.
Separate from my contract are things like a child info sheet, sick policy, a permission form for various things and a few suggestions before starting at my daycare.
