The game was working ok for a few days then a black screen appears after the Activision/Sledgehammer intro. I can hear music but no picture. I did a hard reset on the PS4, checked cords and even got a new disk from eBay games

Looks like other people have had this same issue with the game. There might be something coming up on that forum that will answer your question?  
 
From personal experience: don't let your PS4 go into rest mode either. Just power it all the way down every time. Got lots of weird issues with apps and games after trying to get it out of rest mode. I don't have the pro, though, I have the old PS4.
