Is the customer always right?

It's 2009 and i have been in retail for 20 years and am now in management. If one of my staff sells product to a customer and gets them to sign confirming all colors,measurements,etc. but then when the customer picks it up and it is the wrong size,who should take the blame?
I know you don't order doors all the time but after the order being expained to you about what you're getting,should you not accept this before signing on the line.
What's you're signature worth?
I don't sign my name "on the X" wherever being asked before clearly knowing what i'm signing. What's everyone elses opinion?
