I'm not really sure about the Health Issues. Whether or not with certain health issues a person would have a class one or 2.. Personally I have no health issues but age alone warrants a yearly medical.

Medical re-examinations
Under The Drivers and Vehicles Act the Registrar of Motor
Vehicles requires certain drivers to be medically
re-examined to determine their fitness and ability
to drive. All Class 1, 2, 3 and 4 drivers must submit
a Medical Examination Report form:
• every five years to age 45
• every three years to age 64
• annually at age 65 and over
