Has anyone else received this from Amex?

Today I received an envelope from American Express. Now usually, I don't pay too much attention to this "junk mail" since I get credit card applications and approvals in the mail regularly and they usually just end up in the shredder. I opened it because it had a very non-descript envelope much like another bill I had been expecting.
The letter goes on to say that I "have chosen to enrol in Express Cash" and that I can now use my card to withdraw cash from my American Express Card account at interac abm's in Canada and globally. Included is my randomly selected PIN as well as the "express cash agreement" showing the terms and conditions of the card... the card that I have not received nor have I applied for.
There is no account number anywhere on the document, but it still concerns me that there might be someone out there withdrawing money on this card (up to $1500/mth) if it does, in fact, exist.
Amex wasn't really able to help me without the account number so now I wait to see if I get a bill and then go from there.
